New Forms Effective December 1, 2015: Electronic Filing Unavailable -- (November 16, 2015)

New national forms take effect December 1, 2015. These new forms include the petition, schedules, and statement of financial affairs. You will want to become familiar with the new forms as soon as possible.

On or after December 1st, the link to national forms on the Court’s website [Forms>National] will be updated to connect to these new forms.

These new forms must be used for every case filed on and after December 1, 2015. Failure to use a new form will result in a deficiency notice. If the deficiency is not cured, the document may be stricken, and the case may be dismissed.

For cases filed before December 1st, any document filed on and after January 31, 2016 must use the new form. Again, failure to use a new form will result in a deficiency notice, and failure to correct the problem may result in the striking of the document and the dismissal of the case.

The transition to the new forms will require that electronic filing be closed down briefly. That closure will occur on November 30th from 9:00 PM until midnight. A separate notice about that closing will be posted on the CM/ECF log-in page.
